TravelNurseSource is working with Cynet Health to find a qualified Home Health RN in Hawthorne, New York, 10532 About The Position Job Title: Registered Nurse I Job Specialty: Home Health Job Duration: 13 weeks Shift: Day Shift, Monday to Friday, 8:00 AM to 5:00 PM Guaranteed Hours: Not guaranteed Experience: Minimum of 1 full year of Adult Home Health or Hospice experience License: Must have a current and valid nursing license Certifications: CPR and BLS required Must-Have: - Proficiency in OASIS and experience with HomeCare HomeBase (HCHB) EMR within the last year - Reliable transportation, a current driver's license, and liability insurance - Comfort with on-call duties and weekend rotations - Willingness to travel up to 75 miles from the assigned care center Job Description: - Provide, coordinate, and direct home nursing care according to physician orders and agency policies. - Supervise and evaluate Home Health Aides, Licensed Practical Nurses, and Licensed Vocational Nurses. - Perform home visits and periodic office meetings. - Manage a caseload with an average of 5-8 patients per day. - Complete documentation within 24 hours of patient appointments. - Meet weekly productivity expectations of 30 points, equating to approximately 40 hours per week. - Handle various types of cases, including Start of Care, Wound Care, and Follow-up Visits. - Travel between patient homes, covering approximately 150-200 miles per week, with mileage reimbursed at the IRS standard rate. - Engage in on-call rotations and weekend assignments as needed. - Accept every patient assigned, regardless of travel distance between appointments. 26113978EXPPLAT Job Requirements Required for Onboarding ACLS - AHA BLS - AHA Core Competency Pharmacology Skills Checklist Specialty Exam About Cynet Health Your travel nursing adventure begins here.
