Description:NDHC's AT Home program, is looking for a Full Time (32 Hours) Hospice Social Worker.The primary purpose of this position is to plan, organize, develop, and direct all required social services to meet the needs of residents in accordance with federal, state, and local standards, guidelines and regulations, that govern our facility, and as may be directed by the Administrator to assure that theMedically related emotional and social needs of the resident are met/maintained on an individual basis.

Must possess excellent verbal and written communication skills. - Typical workday hours are Monday through Friday.
