Job Summary:
We're seeking a compassionate and skilled Home Health Aide to provide exceptional care to our clients in the Bronx area. As a CareConnect Home Health Aide, you'll play a vital role in ensuring the safety and well-being of our patients.Key Responsibilities:

• Provide personalized care to patients, including bathing, dressing, and grooming.
• Assist with mobility and transfers, promoting patient safety and comfort.
• Administer medications as directed by healthcare professionals, maintaining patient confidentiality.
• Monitor vital signs and report any changes or concerns to the appropriate medical team.
• Offer emotional support and companionship to patients and their families, fostering a positive care environment.
• Maintain a clean and safe living space for patients, adhering to health and safety standards.Qualifications:
  • Certified Personal Care Assistant (PCA) or Certified Home Health Aide (HHA) credential.
  • Understanding of HIPAA regulations and the importance of patient confidentiality.
  • Ability to cook simple meals and perform light housekeeping duties as needed, promoting patient independence.
  • Satisfactory completion of New York State Health Department-approved HHA Training Program or equivalency.
  • Demonstrated adequate literacy and simple arithmetic skills, ensuring effective communication with patients and staff.
  • Must be at least 18 years old, meeting the minimum age requirement for this role.Benefits:
