This role involves providing and directing nursing and personal care to patients in their homes. Key responsibilities include:

--- Providing high-quality clinical services within infection control standards
--- Completing clinical nursing assessments
--- Ensuring patient eligibility and medical necessity for services
--- Developing and revising individualized plans of care

Experience and licensure requirements include:

--- Minimum one year experience as an RN preferred
--- Current RN licensure in state of practice
--- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
--- Current CPR certification required
