Job Description:

A company is looking for a Care Manager - Complex Care.

Key Responsibilities

Advocates, informs, and educates beneficiaries on services and self-management techniques
Conducts assessments to identify barriers and develops care plans in alignment with treatment goals
Coordinates with multi-disciplinary health care teams to ensure person-centered care and optimal health outcomes

Required Qualifications

New York State Registered Nurse (NYS RN) license
Experience managing large caseloads in a fast-paced environment
Knowledge of community health practices for frail adults and cognitively impaired seniors
Proficiency in using electronic documentation systems and Microsoft Excel
Relevant work experience as a Care Manager is preferred
