Job Description:

Job Summary:We are seeking a highly skilled Registered Nurse to join our team as a Case Manager at Putnam Nursing & Rehabilitation Center. As a RN Supervisor, you will be responsible for organizing and overseeing unit activities, supervising staff assignments, and demonstrating clinical expertise consistent with established nursing standards.Key Responsibilities:

• Organize and oversee assigned unit activities and staff assignments
• Supervise and direct staff in providing high-quality care to residents
• Monitor and respond to patient needs in a timely and appropriate manner
• Administer medications and treatments as needed
• Communicate with interdisciplinary team members, physicians, and other vendors/services to ensure seamless care delivery
• Perform RN assessments and lead emergency situations as neededRequirements:
  • Current and good standing RN license in the State of New York
  • Previous experience in geriatric/short-term rehabilitation/Alzheimer's patients preferred
  • Progressive nursing experience in a geriatric setting, with supervisory experience a plus
